Patient Forms
Upon entering our office,one of our office assistants will welcome you as a member of our practice. We will request that you complete our unique health history profile questionnaire located in the New Patient Center. This provides us with very important information on your condition.
Consultation
Next, you will have a consultation to discuss your health-related problems and concerns. This initial visit is designed for the doctor to learn more about you, your condition, and expectations to determine how Network care can meet your goals.
Examination
After your consultation, a complete evaluation/examination will be performed. State of the art technology including Electrodiagnostic Duel Sensor Imaging and Advanced Biostructural Corrective Digital X-ray will be performed if necessary. However, nothing will be done in our office without your consent.
Report of Findings
Once the information is collected and examinations are performed, Dr. Caliendo will give you a detailed report of all findings and answer any questions including:
- What did you find
- How can you help me?
- How often do I need to come in?
- What will my treatment cost?
After reviewing your health profile, goals, and examining your spine, Dr. Caliendo will discuss recommendations and notify you if your condition requires care with other providers. The doctors and staff will provide the best care and wellness program for your needs.
Practice WellBeing Program
Being well takes practice at first, therefore, a Practice WellBeing program will be suggested to incorporate outside of office visits. If you are in pain when you first come into our office this may include: certain activities or positions to avoid, at home SRI exercises and/or stretches. If you desire, our Practice WellBeing team will work with you also to create healthy habits and routines for your lifestyle. Every person is unique, therefore everyone requires a customized plan. The purpose of our Practice Wellbeing program is for you to achieve and regulate a flexible, healthy spine, have a healthy diet, exercise, and maintain a positive mental state.
